Board Member Skills Evaluation Form
This form should be completed by the individual Board member and represents their view of themselves.

Self and Board Assessment Questionnaire
This form should be completed by the individual Board member and represents their view of themselves and their view of the Board as a whole.

Group Evaluation Form
This form is completed by an observer who is watching the Board in action but is not a Board member themselves ( for example an independent reviewer or the Firms HR Manager).

Board 360 degree appraisal form
This form is completed confidentially by a Board member about all the other Board Members. An independent reviewer or the Firms HR Manager receives the form in confidence and uses the data to report to the Chairman.

Board Member Development Plan
This form is completed by the Chairman when reviewing the Board members performance and delivers agreement with the individual about their development.
